IHS Grant Fund Application 2007

Applications and all supporting documents are required to reach the IHS office by 5 P.M. EDT Friday, August 31, 2007. This information remains here for reference until the next grant cycle.

This page will take you through the process of applying for an IHS grant. Please return to the guidelines page now if you need to review that information first. Following our guidelines closely is your best chance of success.


The application consists of several components, please be sure that you complete and/or attach each:

The application form itself which appears below and then a checklist. Please fill out each field in the form and check each box ( by clicking it) that applies to your group. Your application will not be submitted until we get all of the information that we need from you. Items that are italicized are not required but please include them if they are applicable to your group.

When you complete the process and hit the SUBMIT button, you will receive a feedback page showing the data that was transmitted to the IHS Grant Fund mailbox. If you feel some of this data to be in error, please go back, complete the form correctly, and resubmit it. We will use the later copy.

2. Your Proposal Narrative explaining the projects purpose, activities, and impact (not to exceed 3 pages). See the Proposal Narrative section toward the bottom of the guidelines page for a detailed explanation of narrative requirements.

3. A copy of the government letter confirming your organization's non-profit status (501(c) (3) in the USA)

4. An itemized annual budget for your organization's current fiscal year, with actual figures for the previous fiscal year

5. Your organization's most recent audited financial statement

6. Your organization's latest annual report or summary of prior years activities

7. List of current board members

8. An itemized budget, listing income and expenses, for this specific grant

IMPORTANT: Applications submitted without a Proposal Narrative and other supporting documents will not be considered. Submitting the form below is just one step of the application process. Your application is not complete until all materials have been received by the IHS office.

    

Note: IHS reserves the right to conduct random audits of organizations receiving money from the IHS Grant Fund.
